My Health Toolbox (which tests for and identifies certain medical conditions)
My Health Research (which provides cutting-edge information and options for medical conditions)
My Health Prescription (which reduces company prescription drug cost outlays).

| My Health Prescription

Question: What is the My Health Prescription Program?

Answer: A proven method of reducing your company’s prescription drug costs while improving the overall health and productivity of employees.

Question: You mean it has proven results?

Answer: Yes. Companies which have participated in this program have seen their prescription drug costs drop by half the ($25 pepm [per employee per month]( compared to national formularies ($45).

Question: How much does participation in the program cost?

Answer: $5 per employee per month (pepm), with a minimum one-year commitment.

Question: When can my company participate?

Answer: Whenever it wishes.

| Sequence of Services

Evaluate

My Health Prescription Program administrators will work with you and your Prescription Benefit Manager (PBM) in evaluating your current prescription drug benefits program.

Recommend

We’ll then issue a report which recommends which prescription drugs should be de–listed from (or listed on) your formulary, and why. We’ll also provide you with information you can provide employees who might be affected by formulary plan changes.

Implement

My Health Prescription Program administrators and your PBM (or our recommended PBM, if you prefer) will craft a new formulary, compose notification letters and educational materials (and send them, if you prefer) and prepare you for any appeals which may be submitted.

Report

My Health Prescription Program administrators will issue quarterly and annual reports which measure the baseline and progress/savings of your new formulary.

| Participating in the My Health Research and Assistance Program

Question: How are employees notified of formulary plan changes?

Answer: Via a notification letter and educational materials accompanying the notification letter. Educational materials may include Superior Options documents, Consumer’s Guides and Self-Test Guides.

Question: What if the employee and/or their doctor feels they should continue taking a drug de-listed from the formulary?

Answer: Individuals can appeal a drug’s delisted status and seek its coverage in certain cases. In such appeals, we ask physicians to provide details on why a particular drug is essential in not just treating the symptoms, but also the causes of adverse medical conditions. We also ask doctors to provide information on other treatments used – including diet, exercise and supplementation – which are considered safe. In other words, we need an assurance that every safe, effective method possible has been exhausted before granting appeals on drugs which carry a history of known side effects. Appeals based on these criteria are often granted.

Answer: Participation in the My Health Prescription Program ends upon employee resignation or termination. However, they are free to keep the informational materials they obtained as a participant.

Question: Is the Health Care Toolbox an employee benefit?

Answer: No. It is not tied at all to any benefits program, and is being made available to any company regardless of their benefit offerings. It is a pre-paid wellness program -- a service employers can provide their employees and their immediate family members.

Question: Who can participate in the Health Care Toolbox?

Answer: All full- and part-time employees, their spouses and dependants (16 years old and older).

Question: How much does participation in the Health Care Toolbox cost?

Answer: $150 per participant

Question: Does the $150 participant fee cover doctors' visits, supplements, supplies, etc?

Answer: No. Employees pay for doctor's consultations, non-Toolbox tests, services, supplies, supplements and other materials purchased by the employee on their own.

Question: When can my company participate?

Answer: Whenever it wishes, as frequently as it wishes. We recommend at least twice a year. That level of frequency means employees will have timely information about such things as their cholesterol, blood pressure and glucose levels -- as well as their overall health -- and allow them to take corrective measures in a proactive manner, if necessary.

| Sequence of Services

PHASE 1: REGISTER (THREE WEEKS)

Employers are sent informational packets about the Health Care Toolbox. Employees (and their spouses and dependents 16+; also known as "participants") can then complete either online or hard–copy registration forms, which are sent to Health Care Toolbox administrators for processing.
Participants are sent confirmation of their registration via e–mail or hard copy.

PHASE 2: TEST (SIX WEEKS)

Along with their confirmation notification, participants receive:

A pH test, which can be completed anytime at home, providing basic information on the participant's pH balance.
A web address and temporary username and password enabling them to complete an online Health Risk Assessment (or HRA; hard copy HRAs are available upon request). HRAs should be completed within two weeks.

Based on the number of registrants, Health Checks (combined blood pressure screenings and blood draws) are scheduled to be held at the work site approximately one month after all participants have registered.
After the Health Checks are completed, Health Care Toolbox administrators receive each participant's Health Check results and integrates that data into the completed HRAs.

PHASE 3: GET INFORMED (2–3 WEEKS)

Health Care Toolbox administrators then print the individual HRA reports and blood test results and send them to each participant in sealed, confidential envelopes.
All online HRA reports can be archived online and accessed anytime by participants using their username and password. Future HRAs can be compared alongside previous ones for tracking of progress.
Concurrently, Health Care Toolbox administrators will produce a "Productivity and Economic Benefits" report by location (as well as company–wide) summarizing the risk factors exhibited by participants, as well as an estimate on the amount of money saved in lost productivity, absenteeism, etc. by adopting healthy lifestyle habits.
Participants are sent basic information about the top health conditions plaguing Americans, including (but not limited to) health coach referrals, newsletters, superior options documents and Hyperhealth reports.
Shortly after all participants have received their reports, a Health Care Toolbox Health Coach will hold a one–hour "Lunch 'n Learn" at the workplace to answer questions about blood tests, Health Risk Assessments, etc. as well as address the most significant conditions plaguing the client company.

PHASE 4: IMPLEMENT

Participants then implement the recommendations mentioned in Phase 3 ("Get Informed"). This may include diet, exercise, sleep and supplementation suggestions.

PHASE 5: RE–TEST (6 MONTHS TO ONE YEAR LATER)

Participants then re–enroll for a renewed round of testing, enabling them to obtain updated biometric, blood, pH and HRA data.
Based on new data, participants can make needed lifestyle adjustments based on the Phase 3 ("Get Informed") information sent to them.

Question: ...a participant can't attend the worksite Health Check, or they work in a remote location?

Answer: Please inform your Health Care Toolbox administrators. They will make arrangements for the participant to receive all Health Check offerings.

These medical professionals will often spend more time with you than a regular doctor, and will focus on treating the causes, not just the symptoms, of your medical conditions(s).

Question: Will the employer ever see employee medical data?

Answer: Never. We take great precautions to ensure only the participant knows about their health conditions. Health Care Toolbox and the employer may choose to obtain general information about health conditions in the company (to aid in selecting content for health communication pieces, as well as for the company's Productivity and Economic Benefits Report), but this information does not contain names, birthdates or any other identifiable information.

Answer: Yes. Once you are enrolled by your employer, My Health Research and Assistance Program administrators will work with you in researching effective treatments for your spouse or dependent 16+ with an adverse medical condition.
